Bridging the Divide: Primary Care and Public Health Conference

November 4, 2016
The Essex, Essex Junction, VT

For the past seven years the Bridging the Divide conference has focused on enhancing collaboration between Primary Care and other disciplines such as Mental Health, Pharmacy, and Oral Health. This year we explore the myriad connections between Primary Care and Public Health. As our health system moves from a fee-for-service payment model to one focused more on value-based payment and overall population health, the linkages between Primary Care and Public Health have never been more important.

The goal of this conference is to bring together health care and public health professionals to learn more about their respective points of view and priorities.
